These Terms of Service ("Terms") govern your use of Dose Harbor Clinic, operated by Dose Harbor Clinic ("we", "us", or "our"), including this website, its checkout, and your patient account (together, the "Services"). By creating an account or placing an order you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Services.
You must be at least 18 years old, located in the United States in a state we serve, and ordering for yourself. We currently serve patients located in the United States.
Accounts are personal. Sign-in is by a one-time code sent to your email address, and you are responsible for activity under that address. Do not share your sign-in codes.
Prescription products require review and approval by a licensed healthcare provider before they can be fulfilled:
Approval is not guaranteed. The provider's decision is an independent clinical judgment. Do not rely on a pending order for any health need.
Content on this site is for general information only and is not medical advice, diagnosis, or treatment. Individual results vary. Only the licensed provider who reviews your request can advise you about your care.
This is not emergency care. If you have a medical emergency, call 911 or go to the nearest emergency room. Do not use the Services for urgent needs.
Approved prescriptions are filled and shipped by a licensed U.S. pharmacy that ships to your state.
Providers may prescribe compounded medications when appropriate for your needs. Compounded medications are prepared by a licensed pharmacy for an individual patient. The U.S. Food and Drug Administration does not review or approve compounded medications for safety or effectiveness. Actual product packaging may differ from images shown.
Prescriptions are issued at the provider's sole discretion, are not transferable, and may be limited in quantity or refills.
Prices, treatment plan terms, shipping options, and any fees are shown before you pay. Payment is processed by our payment processor; we never store your full card number.
When you place an order, your payment method is authorized for the first payment of the plan you chose. You are charged only when a provider approves your request. If your request is declined, the authorization is released and you are not charged for it. Authorizations that are not captured may take several business days to disappear from your statement, depending on your bank.
Treatments are sold as treatment plans. The plan's length, the number of shipments, the price of each billing cycle, and whether you pay per cycle or up front are shown before you pay and again on your order confirmation.
Standard shipping is included in your plan price. Where offered, you may choose a paid faster shipping option at checkout; the upgrade applies to that shipment only. Delivery times are estimates and depend on the pharmacy and carrier. Medications ship only to the address you verified at checkout and cannot be redirected in transit.
Refunds, when issued, go back to the original payment method.
Your account shows your orders, treatment plans, and shipments, and lets you message us about your care. Messages are part of your medical record. Do not use messaging for emergencies. We may close accounts that are inactive, used fraudulently, or in violation of these Terms.
By placing an order you agree to receive automated transactional text messages, calls, and emails about your order and care. Marketing messages are sent only if you separately opt in, and consent is not a condition of purchase. Message frequency varies, and message and data rates may apply. Reply STOP to stop texts or HELP for help. See our Privacy Policy for details.
Do not misuse the Services: no unauthorized access, scraping, interference with operation, false identities, or unlawful use. We may suspend or close accounts that violate these Terms, and we may decline to serve you where permitted by law.
The content, design, and trademarks on this site belong to us or our licensors. You may use the site for your personal, non-commercial use only.
THE WEBSITE, CHECKOUT, AND PATIENT ACCOUNT ARE PROVIDED "AS IS" AND "AS AVAILABLE". TO THE EXTENT PERMITTED BY LAW, WE DISCLAIM ALL IMPLIED WARRANTIES, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT, WITH RESPECT TO THE WEBSITE AND ITS OPERATION. NOTHING IN THESE TERMS LIMITS OR DISCLAIMS THE PROFESSIONAL DUTIES OWED TO YOU BY A LICENSED PROVIDER OR PHARMACY, OR ANY RIGHTS YOU HAVE UNDER CONSUMER PROTECTION LAW THAT CANNOT BE WAIVED.
TO THE EXTENT PERMITTED BY LAW, OUR AGGREGATE LIABILITY ARISING FROM YOUR USE OF THE WEBSITE, CHECKOUT, OR PATIENT ACCOUNT (AS DISTINCT FROM THE MEDICAL CARE YOU RECEIVE) IS LIMITED TO THE GREATER OF $100 OR THE AMOUNTS YOU PAID THROUGH THE SERVICES IN THE 12 MONTHS BEFORE THE CLAIM. THIS SECTION DOES NOT LIMIT LIABILITY FOR GROSS NEGLIGENCE, WILLFUL MISCONDUCT, PERSONAL INJURY CAUSED BY NEGLIGENCE, PROFESSIONAL NEGLIGENCE, OR ANY LIABILITY THAT CANNOT LAWFULLY BE LIMITED.
These Terms are governed by the laws of the state in which Dose Harbor Clinic is organized, without regard to its conflict-of-law rules, except where the consumer protection law of your state applies and cannot be waived. Any dispute that cannot be resolved informally will be brought in the state or federal courts located in that state, and you consent to their jurisdiction. Before filing a claim, contact us (Section 18) so we can try to resolve it with you.
We may update these Terms. The current version is always posted on this site with its "Last updated" date. Material changes take effect when posted and, where required, when you accept them again at your next sign-in or checkout. Continued use of the Services after changes means you accept them, where permitted by law.

Your payment purchases a 3-month treatment bundle: 3 patient-specific vials of compounded medication (a 3-month supply), all injection supplies, shipping, and a professional medical evaluation by a licensed clinician. Pricing is flat and dose-independent - every 3-month Compounded Semaglutide bundle is the same price at any dose (Dose 1-5), and every 3-month Compounded Tirzepatide bundle is the same price at any dose (Dose 1-6). Your price never changes because your dose changes. Please read this section carefully, because it explains exactly what you are buying.
There is no guarantee of approval. Paying for a bundle does NOT purchase a prescription or any particular outcome. A licensed clinician will review your health information and independently decide whether the requested treatment is safe and appropriate for you. Possible outcomes include approval, approval with conditions, a request for more information or a live video visit, or a decline. Medication is prescribed, prepared, and shipped only when the clinician determines it is clinically appropriate. We explicitly do not promise, guarantee, or imply that you will be approved or prescribed anything. Any marketing you may have seen refers to the availability of the bundle and evaluation, not to the evaluation's outcome.
Refunds. If the evaluating clinician determines that treatment is not appropriate for you and your case is declined, your bundle payment is refunded in full to your original payment method. If you cancel before a clinician begins reviewing your case, you may request a full refund. If your visit cannot be completed for reasons on our side - for example, no licensed clinician is available in your state, or a required live video visit cannot be scheduled within a reasonable time - you will be refunded in full. Once your case is approved and your medication has been prepared and shipped, the bundle payment is earned in full; compounded, patient-specific medication cannot be restocked or resold, so shipped bundles are not refundable except as required by law or where the product arrives damaged or unusable.
Payment disputes and chargebacks. When you sign, we record your typed legal name, the exact date and time of signing, your IP address, and your device information, along with the versions of the documents you agreed to. This signed record demonstrates that you understood, before paying, what the bundle includes, that approval is not guaranteed, and that a declined case is refunded in full rather than shipped. If you dispute a charge with your card issuer, we will respond with this record. Please contact support before initiating a chargeback - most concerns can be resolved directly.
By signing, you confirm that you have read these terms, that you understand the bundle price is flat regardless of dose, that a prescription is issued only when clinically appropriate, that approval is not guaranteed, and that you agree to these payment and refund terms.
